An exciting opportunity is available within the Huntingdon Learning Disability Team for an Adult Support Co-ordinator, 37 hours per week.
We are offering the opportunity to work within a unique model of practice as you will have the opportunity to practice as part of an Integrated Team alongside Health and Social Care Practitioners such as Adult Support Co-ordinators, Social Workers, Community Nurses, Occupational Therapists, Physio Therapists, Psychologists, Psychiatrist, Speech and Language Therapists and Art / Music Therapists.
This position will suit someone who has the skills and knowledge to support the needs of Adults with Learning Disabilities and their carers or can offer transferable skills and experience that would be utilised in this role.
We offer regular supervision and training/development opportunities and opportunity to develop interest areas within the partnership.
As the successful candidate you will work with adults with Learning Disabilities living in different settings such as their own homes, supported housing, residential care homes and respite care. you will also actively assist and provide advice to carers, conducting carer’s assessments, as required by legislation, to enable them to sustain their caring role.
Your key duties will include undertaking and completing Care Act compliant Social Care Assessments, reviews, arranging packages of support and using Direct Payments and Arranged Provisions.
You will manage a varied caseload and support the team in delivering the First Response (Duty) function, work under the Transforming Lives approach and be proactive in building links with local communities. You will need have the knowledge to identify safeguarding concerns, liaise with the Safeguarding Leads to determine the appropriate response and support the agreed action plan.
Your office base will be Scott House in Huntingdon.
